MASH and Cardiomyopathy Targets: Why AstraZeneca Chose insitro’s AI
MASH and cardiomyopathy AI collaboration: AstraZeneca pays insitro $40M upfront for target discovery, then leads clinical development and CMC planning.
AstraZeneca and insitro announced on Oct. 15, 2026, a strategic drug discovery collaboration and licensing agreement that will apply insitro’s Virtual Human AI platform to identify novel drug targets in metabolic dysfunction-associated steatohepatitis (MASH) and cardiomyopathies.1,2 Under the terms, insitro will receive $40 million upfront and is eligible for up to approximately $1.46 billion in potential development and commercial milestones, plus potential single-digit royalties on global commercial sales. AstraZeneca will hold exclusive worldwide rights to advance programs selected from the collaboration.
The division of labor is clearly defined.1,2 Insitro will identify causal disease biology across both indications and advance AstraZeneca-nominated targets from discovery through to development candidate. AstraZeneca will lead all clinical development, regulatory work, and commercialization.
Why Apply AI to Heterogeneous Diseases Like MASH and Cardiomyopathies?
Both indications share a defining problem: a single diagnostic label covers patients whose underlying biology differs, and that heterogeneity has made the diseases difficult to treat.2 MASH affects more than 250 million people worldwide.
The insitro platform combines large-scale human and cellular data with AI-enabled target identification and medicine design.2
“MASH and cardiomyopathies are prevalent, underdiagnosed diseases where high-content data holds immense signal for uncovering disease drivers. By applying AI to these complex datasets, we can unlock greater discovery power and reveal novel genetic insights. Our platform simultaneously deconvolutes disease biology and identifies causal drivers of disease pathologies. Partnering with AstraZeneca allows us to pair our deep biological precision with their deep pharmaceutical expertise and scale with potential accelerated advancement of novel programs through development and to patients,” said Daphne Koller, PhD, founder and CEO, insitro, in a press release.2
For AstraZeneca, the agreement extends a series of AI-focused discovery partnerships, including a
“We know that disease biology is incredibly complex, and one of the most critical decisions in R&D is selecting the right target. This strategic collaboration underscores our commitment to maximising the potential of cutting-edge AI to tackle the root cause of disease. Partnering with insitro, we will use advanced AI to identify the biological drivers of disease to support the rapid discovery of high-quality novel targets, advancing our ambition to deliver the next generation of medicines to patients faster,” said Sharon Barr, head of BioPharmaceuticals R&D, AstraZeneca, in the press release.1
What Does the Deal Mean for Development and Manufacturing Teams?
Discovery-stage agreements rarely change operations immediately, but the structure carries downstream implications.1,2 Target selection is among the earliest decisions shaping total development cost, and stronger causal evidence at the outset could reduce late-stage attrition, which strands investment in process development, scale-up, analytical methods, and clinical supply.
The handoff point is relevant because insitro’s responsibility ends at development candidate, chemistry, manufacturing, and controls planning.1,2 Formulation, and supply strategy will sit with AstraZeneca.
The deal also highlights an industry imbalance of AI investment being concentrated in discovery while downstream execution lags.1,2 As
